You're correct that those *are* three ways that you could explain a high rate of communication failures without reference to your intrinsic skill, but you then jump to "so the problem is not my skill" without exploring that branch.
My guess, knowing nothing else, is that both are true -- you have some behavior pattern that manifests as communication failures sometimes, *and* you provide more opportunities for it to manifest.
